Live Healthy Miami Gardens
The Live Healthy Miami Gardens (LHMG) Initiative is part of the City of Miami Garden’s strategic and systemic effort to reduce poor health outcomes by engaging community residents to improve selected public health indicators. LHMG was built upon the understanding that no single organization can solve tough problems on its own and that community involvement is essential to reducing health disparities. LHMG is a Collective Impact initiative bringing people together and strengthening the community’s capacity to collaboratively plan and collectively carryout strategies that make the vibrant community of Miami Gardens healthier.
The City of Miami Gardens in partnership with the Health Foundation of South Florida (HFSF) launched LHMG in February 2014. After an extensive and highly competitive process led by the Foundation, the City was selected as a grantee to establish and lead LHMG through a six-year, $3.75 million dollar grant. The initiative is now also funded by the National League of Cities (NLC) and the Centers for Disease Control and Prevention (CDC), Racial and Ethnic Approached to Community Health (REACH).
Federation of Families
Federation of Families, Miami-Dade Chapter, Inc., (FOF) is a 501C-3 non-profit organization located at its Youth & Family Center in Miami Gardens, Florida. Since 2010, Federation of Families has provided a holistic approach to physical and mental well-being for families and the community, through alternative/enrichment activities and education.
Its Triple P Parenting programs prioritize the social-emotional education of parents alongside their children and teenagers to create more stable home environments that lay the foundation for a healthy life.
FOF also provides training and education that builds mental health awareness and empowers participants to take charge of their well-being. The Youth Mental Health First Aid (YMHFA), Adult Mental Health First Aid (AMHFA), and the Wellness Recovery Action Plan (WRAP) Seminar are the three most requested courses provided.
In addition, FOF provides alternative and enrichment activities to youth and families. These programs are designed to create positive and alternative ways to self-regulate individual emotions and behaviors by offering participants of all age’s various creative outlets. Some examples of activities provided are art for emotional regulation sessions, creative expression through art, yoga, mindfulness and self-care activities that improve quality of life and create healthy habits.

Optimal Health Medical Center (OHMC) is an all-inclusive medical facility providing primary care, senior care, and pharmacy services, located in the heart of Miami Gardens. OHMC follows a holistic model grounded in providing preventative care education and access to social supports. Our mission is to promote the overall health and well-being of our patients by providing quality and comprehensive healthcare services. OHMC is committed to preventing and managing chronic disease by exploring and providing solutions to the root causes that negatively impact health. We aim to empower individuals to reach their best health through preventative care education and linkage to socioeconomic resources.
Fresh Start
Since 2000, Fresh Start of Miami-Dade, Inc. provides community Adult Mental Health services such as self-help, education, training, relaxation, recovery through the arts, recreation activities while promoting advocacy and empowerment in a peaceful, homelike, nurturing and supportive environment for those with mental health and co-occurring challenges.
The agency reaches out to help its consumer-based membership and the community by building strong relationships, building strong advocacy and active consumer participation – the vehicle for combating isolation, hopelessness, and nurturing and building support networks. The services provided enhance and maintain recovery, personal dignity and offer an environment that protects personal privacy while maintaining the personal integrity of its membership. Self-determination for each member is the agency’s priority and it ensures that members retain the fullest possible control of their lives and as much as is possible, be able to set their own goals and participate in planning the activities surrounding their own lives.
University of Miami
Department of Education and Human Development
The University of Miami (UM) and specifically its School of Education and Human Development and School of Nursing and Health Studies, are ideal locations for a well-developed and culturally informed effort to train medical, nursing, and counseling students and personnel on SBIRT. UM is located in one of the most ethnically diverse areas in the U.S. and is in close proximity to Latin America and the Caribbean. UM ranks third in the nation among private research universities for percent of minority students at the undergraduate and graduate levels. The University capitalizes on the representation of ethnic minority groups in Southern Florida, including Miami-Dade and Broward Counties, so as to have an increased impact on the health care system that serves a diverse population. UM ranked 54th of all universities in expenditures of federal funds for research and development. This means that it has the type of infrastructure and assistance that helps ensure that projects can be implemented as proposed. While the UM SBIRT training team goes out to provide training in community practice centers, training also occurs in the classrooms, reaching future healthcare professionals early in their training.
